Accessing ALARA Services

People with a disability and seniors wishing to obtain information about our Disability Direct Services, NDIS Support Coordination, NDIS Plan Management NDIS Plan Management or Aged Care Services can contact the ALARA Intake Officer on 07 3817 0600 between 8.30am to 4.30pm Monday to Friday (excluding public holidays).

If you are specifically interested in centre-based services, you can contact the Centre Coordinator directly at Luke’s Place Ipswich Area on 07 3817 0600, Luke’s Place Esk (on Tuesday, Thursday and Friday only) on 07 5424 1242, or Luke’s Place Lockyer 07 5465 3742.

If you are only interested in attending the Activities program, you can speak directly to the Activities Coordinator on 07 3817 0616.

The following programs offered by ALARA are accessed through an external assessment and referral agencies: 

Aged Care - Commonwealth Home Support Program (CHSP) and Home Care Packages (HCP)

To apply for an assessment for access to government subsidised Aged Care services, you or your representatives can complete an assessment on-line at https://www.myagedcare.gov.au/assessment/apply-online.

If you need some help, the My Aged Care team can provide assistance over the phone. Information and support is available to help people of all backgrounds, languages and situations to access the help they need.

Assessment for eligibility for Aged Care Services is undertaken by RAS or ACAT assessors who after completing the assessment, refer the client to services delivered by approved service providers. ALARA accepts clients through the My Aged Care Portal where we have capacity or directly where the client has the referral code from My Aged Care.

Queensland Community Support Scheme

All new applicants for QCSS supports must contact the free telephone and web-based portal, state-wide QCSS Access Point. To apply, you can phone the Queensland Community Support Scheme Access Point on 1800 600 300 or complete a self-registration form.
